Childhood Vaccination Workflow: During Office Visit

For all children, preteens, and teens

Patient completes vaccine questionnaire at appointment.

Immunization records may be found in your EHR, IIS or the parents’ or caregivers’ personal records. Use your EMR, IIS, and/or current ACIP Recommended Child and Adolescent Immunization Schedule to determine what vaccines are needed.

Screen for contraindications and precautions.

Use the Screening Checklist.

Educate the parent/caregiver about needed vaccines.

Provide parents/caregivers with the Vaccine Information Statements (VIS). Answer any questions that the parent/caregiver may have. Federal law mandates that a VIS must be given to parents/caregivers before administering the vaccine(s) to the child.

Parent/Caregiver Says "Yes"

Parent/Caregiver Says "No"

Prepare vaccine(s).

Follow office protocol to encourage vaccination.

Administer vaccine(s).

Bill for time spent on vaccine counseling per protocol.

Document vaccine(s).

Bill for vaccine administration.
